		Scheduling Appointments To Schedule an Appointment for your Back or Neck Condition: Contact the office at (425) 454-1111 between 8:30 am and 4:30 pm Monday through Friday. Expect the scheduling process to take at least 10 minutes. Not only do we need to ask you several questions, but we want to have adequate time to address your concerns. Have your referring physician complete a referral form (PDF). Complete a registration form (PDF) or have all insurance and/or claim information available. How to Prepare for your Appointment at the Surgery Center: Carefully read the Pre-Procedure Instructions (PDF) and follow all the instructions. Do not forget to bring all related diagnostic films/CDs (x-rays, MRIs, CT scans, SPECT scans). Bring a valid insurance card and driver’s license. Download the Procedural Information regarding the injection you are scheduled to receive. If time allows, you can download, print and complete the Pre-Procedure Questionnaire (PDF). You will then have one less form to complete at the surgery center.
